SCENARIO 1: Client is on a gluten-free diet without confirmed diagnosis.
Client should complete a gluten challenge for 6-8 weeks followed by scenario 2.
SCENARIO 2: Client is on a gluten-containing diet.
- Order celiac serology. If normal total IgA and elevated anti-tissue transglutaminase IgA they likely have celiac disease.
- Order endoscopy including 5-6 biopsies. If pathology shows increased intraepithelial lymphocytes and villous blunting, celiac disease may be confirmed.
